Agape Counseling Center - Make an Appointment
Thank you for visiting, if you would like to make an appointment with one of our mental health professionals, please follow the steps given below to schedule your first session.
- Call us at 937-434-0540 during office hours, have your insurance information on hand.
- The friendly staff here will check your insurance information and call you back with financial information such as your applicable deductible and expected co-pay.
- Based on your information and needs, the staff will schedule you for an appointment with the appropriate clinical staff
- Please arrive approximately 20 minutes before your scheduled appointment time to check-in and complete in remaining intake information

Items to bring with you on your first visit:
- Your insurance ID Card.
- A list of your current prescription medication.
- Contact information for your Primary Care Physician, including their name, office address, and phone number.

Based on your applicable deductible, and/or co-pay amount. Please be prepared to pay your applicable amount at your first appointment.
- If for some reason you feel you will be unable to pay the full amount for your session, please call our office to discuss what payment options might be available to you.
